The HAP Health Engagement Program concentrates on seven important health measures:
HAP chose these measures because you can control them through smart choices and they have an impact on your health and your risk of developing chronic diseases.
Your physician will determine if you meet the wellness target for each health measure. A score of 80 points or more each for both you and your spouse qualifies you for the Enhanced plan.
