Wellness Targets

The HAP Health Engagement Program concentrates on seven important health measures:

  • Tobacco Use
  • Weight
  • Blood Pressure
  • Cholesterol
  • Blood Sugar
  • Alcohol Use
  • Preventive Tests

HAP chose these measures because you can control them through smart choices and they have an impact on your health and your risk of developing chronic diseases.

Your doctor will determine if you meet the wellness target for each health measure. A score of 80 points or more each for both you and your spouse qualifies you for the Enhanced plan.

Health Measure Wellness Targets What can I do to qualify for Enhanced benefits if I do not meet the wellness target?
Tobacco Use
(25 points)
Nonsmoker Agree to a smoking cessation treatment plan.

Healthy Weight
(15 points)

Body Mass Index at or below 30
Calculate your BMI
Agree to participate in a weight management program.
Learn more about HAP’s weight management programs.
Blood Pressure
(15 points)
At or below 140/90 Agree to follow your PCP’s treatment plan.
Cholesterol Control
(15 points)
Low density lipoprotein (LDL-C) at acceptable level (based on risk factors) Agree to follow your PCP’s treatment plan.
Blood Sugar
(10 points)
Normal blood sugar or acceptable hemoglobin A1C Agree to follow your PCP’s treatment plan.

Alcohol Use
(10 points)
Consume less than 2 drinks per day on average and pass a physician-administered screening exam. Agree to follow your PCP’s treatment plan.
Preventive Tests
(10 points)

All age/gender specific tests are current Agree to obtain all tests within recommended timeframes.
