Health Engagement Program

Good health should be rewarded. So we designed HAP's Health Engagement Program to cost less for people who make healthy choices.

The Health Engagement Program provides you with 2 benefit plans: Enhanced and Standard.

For the first 90 days of coverage, you and your covered spouse will be placed in the Enhanced plan with lower out-of-pocket costs. To stay in the Enhanced plan, you and your covered spouse will need to do the following within 90 days of your contract effective date:

  1. Complete the online iStrive Succeed Health Risk Assessment.
  2. Meet with your personal care physician (PCP) to complete the Member Qualification Form and return the form to HAP before the end of the qualification period. Remember: Results from lab work completed in the last 9 months qualify, but your physician must complete and sign the form. Physicians can now complete and submit the form online. Please retain a copy for your files or access it online by logging in at www.hap.org. See FAQs for more information on how to access your Member Qualification Form online.
  3. Achieve specific wellness targets or commit to participating in health improvement programs or treatment plans. You and your covered spouse must score at least 80 out of 100 possible points to qualify for the Enhanced plan. Learn more about wellness targets and what to do if your score is less than 80.

At the end of the 90-day period, if you and your covered spouse have not completed these steps, you and your family will be placed in the Standard plan for the next 12 months. With the Standard plan, you have the same access to quality care and physicians, but you will have higher out-of-pocket costs.
